Test Proctor Request Form

This form must be completed only by instructors.

Students must have received the Accommodation Memo from the Counseling and Student Disability Services (CSDS) for the current term and present the memo to the instructor at least five days before receiving testing accommodations.

It is the responsibility of the instructor and student to work together to accommodate a student. Counseling and Student Disability Services (CSDS) provides the option for students to test in our offices as a courtesy to faculty who are unable to proctor. Our office needs the Test Proctoring Request Form, submitted a minimum of  3 business days in advance in order to proctor for the student. The test will be proctored within our office hours, Monday–Friday, 8:00 AM–5:00 PM.  Using Counseling and Student Disability Services (CSDS) outside of these hours will need to be discussed on a case-by-case basis; faculty should contact Counseling and Student Disability Services (CSDS) with the specific time of the exam to see if alternative arrangements can be made. Faculty members should consider being available by telephone or e-mail during the test in case the student with the disability requires clarification on any aspect of the test. Please provide the best contact method to the student when completing the test proctor request form.

Counseling and Student Disability Services (CSDS) test proctoring is monitored by using cameras.

NOTE: If space is unavailable in an alternative testing location during the needed time frame, faculty and students must make other arrangements so that the student still receives the accommodations, however, both student and instructor must agree to the alternative test site.

Once the request and test information have been submitted, your student(s) must contact their counselor to schedule a test proctoring appointment.

Due to COVID-19, CSDS is currently operating by virtual appointments, phone, and email. Instructors must proctor their own exams while ensuring that student receive their approved accommodations. Proctoring may be facilitated by CSDS on a very limited case by case basis. Please contact our department 5 business days in advance so that we may discuss your proctoring request.
